Recommendation from the concerned Ward Office on A4 size Nepali paper, filling Schedule-7, certified by the Ward Chairperson with a recent passport-sized photograph.

Citizenship certificates of the father and mother, indicating their country of origin/identity.

Original marriage registration certificate and affidavit (Sapath Patra).

Proof of identity of the concerned country and documentation/proof of initiating the process to relinquish foreign citizenship.

The husband who verifies the application (Sanakhat) and his original Nepali Citizenship Certificate.

If necessary for verification, a police and local level (Local Body) verification report (Sarjamin Muchulka).
