Application form filled with details as per Schedule-1 of the Regulations, with the photo also certified by the concerned Ward Chairperson.

Mother's Nepali Citizenship Certificate.

Birth Registration Certificate and Educational Qualification Certificate.

Recommendation and Verification Report (Sarjamin Muchulka) issued by the concerned local level, stating the place of birth, relationship with the mother, that the applicant was born in Nepal and has been residing in Nepal, and that the identity of the father could not be ascertained.

Two copies of a recent passport-sized photograph showing the full face and both ears.

Verification (Sanakhat) by the mother.

Self-declaration (Swaghoshana) as per Schedule 4 (a) and 4 (b) of the Regulations.

If the mother is deceased, the death registration certificate. If the mother is of unsound mind (not in her right senses), a recommendation from an approved physician.
