Application Form: Application form filled with details as per Schedule-1 of the Regulations, with the photo also certified by the concerned Ward Chairperson.

Institutional Recommendation: If raised in a Government-recognized institution like a child welfare home (Baal Mandir), orphanage (Anathalaya), etc., a recommendation letter from that institution, along with verification (Sanakhat) by the head of the institution, and a recommendation and verification report (Sarjamin Muchulka) prepared by the concerned local level based on this.

Legal Guardian's Verification: If raised by a legal guardian, a verification (Sanakhat) by that guardian, their Citizenship Certificate, and a recommendation and verification report (Sarjamin Muchulka) from the concerned local level.

Individual Caregiver's Verification: If raised by a specific individual, the Nepali Citizenship Certificate and verification (Sanakhat) of that caregiver, along with a recommendation and verification report (Sarjamin Muchulka) from the concerned local level confirming the same details.
