Application Form: Application form filled with details as per Schedule-1 of the Regulations, with the photo also certified by the concerned Ward Chairperson.

Photographs: Two copies of a recent passport-sized photograph showing the full face and both ears.

Parents' Citizenship: Nepali Citizenship Certificates of both the father and the mother (Birth-Based Citizens).

Proof of Birth: Birth Registration Certificate of the applicant.

Local Recommendation: A recommendation letter from the concerned local level confirming the applicant's place of birth and lineage (Naata).

Educational Proof: Educational Qualification Certificate.

Migration Proof (If Applicable): Migration Registration Certificate (Wasaai Saraai Darta Praman Patra) if applying for citizenship after changing the address/residence.
